BACKGROUND:

Tuberous breast syndrome was first described in 1976 by Rees and Aston, and incidence seems increasing worldwide. This congenital malformation includes several types of abnormalities, the most constant of which is a defect in the development of the breast base, appearing in adolescence. Nowadays, this deformity is surgically corrected mainly using breast implant in association with dermo-glandular flap and corrections of areolar protrusions if needed. The use of fat graft in breast surgery and more particularly in tuberous breasts makes lipofilling considered as a new paradigm shift The aim of this study was to evaluate the long-term results of tuberous breast corrections: lipofilling versus implant, focusing on patient satisfaction.

MATERIALS AND METHODS:

Thirty-nine patients, who underwent correction of tuberous breast deformity from 2002 through 2018 were considered. Twenty patients in the lipofilling group and nineteen patients in the breast implants group. Age, degree of deformity, asymmetry, BMI, complication, pregnancy and further surgical procedures were evaluated; Patient satisfaction was evaluated with BREAST-Q and statistical analysis was conducted to compare it.

RESULTS:

BREAST-Q evaluation suggested that patient satisfaction was comparable at long term follow up (>5years). A question-by- question analysis revealed several significant differences leading to interesting discussions.

CONCLUSIONS:

Our long-term follow-up and the BREAST Q responses make us think that lipofilling in tuberous breast deformity is a technique especially applicable to young teenage girls who suffer from lack of confidence due this breast deformity, but for whom the placement of implants or use of flaps would be premature.
