Premières représentations de l'oralité dans les coordinations oeil-main-bouche chez l'enfant de 0 à 6 mois à travers les neurosciences et les stratégies neuronales au niveau de la jonction oeil-main-bouche

L’oralité se développe in utero. Les premiers contacts foetaux génèrent des afférences sensitivosensorielles vers les réseaux d’interneurones para-solitaire et para-ambigu du tronc cérébral, formant les central pattern generators de succion et de déglutition. Après la naissance, l’oralité sera progressivement sous contrôle cortical. Les mouvements associés entre l’oeil, la main et la bouche se développent également in utero et évoluent après la naissance vers une coordination plus corticale, liée au comportement alimentaire. Le réseau neuronal dédié à l’alimentation est intimement lié au circuit de la récompense. Les expériences variées du nourrisson dans ses interactions avec l’environnement lui permettent de sélectionner et d’entretenir les réseaux neuronaux offrant les comportements les plus adaptés. Le lien entre la coordination oeil-main-bouche et le réseau dédié à l’alimentation permet de faire l’hypothèse que les contacts main-bouche répétés lors des expériences motrices du bébé seraient préservateurs du réflexe nauséeux anormal. Cependant, elles sont limitées par l’installation contraignante (cosy, …). L’évaluation des contacts main-bouche dans différentes installations ne peut être réalisée par l’évaluation qualitative des General Movements. De nouveaux outils et critères d’analyse des mouvements du bébé doivent donc être développés. L’objectif de cette étude est de valider l’utilisation du logiciel ELAN dans cette évaluation et de développer de nouveaux critères d’analyse. 5 bébés entre 9 et 16 semaines ont été filmés en motricité libre pendant 15 minutes dans cinq types d’installation. Ces vidéos ont été analysées indépendamment par deux examinateurs et réanalysées par chacun des examinateurs à distance. La fiabilité des résultats des analyses vidéo inter-examinateur et intra-examinateur obtenus avec ce logiciel a été vérifiée. ELAN est un outil simple et fiable d’analyse des mouvements de l’enfant. Il sera prochainement utilisé pour comparer les premières représentations de l’oralité dans différentes installations.

Orality develops in utero. First fetal contacts generate sensitivo-sensory afferents to para-solitari and para-ambiguus networks of interneurons in the brainstem, forming the central pattern generators of sucking and swallowing. After the birth, orality will be gradually under cortical control. The movements between the eye, the hand and the mouth develop in utero, too, and advance to more cortical coordination after the birth. This one is linked with feeding behavior. As a matter fact, the network used for the feeding behavior is tighly linked with the reward circuitry. The varied experiences of the infant in its interactions with the environment allows to select and maintain the neural networks providing the most adapted behaviors. The link between the eye-hand-mouth coordination and the network used for food enables to suppose that repeated hand-mouth contacts during the motory experiences of the baby would protect it from the abnormal gag reflex. However, they are limited by children seats. The evaluation of hand-mouth contacts according to these seats can’t be achieved thanks to the qualitative assessment of the General Movements. New tools and criteria analysis of the baby’s movements must therefore be developed. The purpose of this study is to validate ELAN software utilisation in this evaluation and to develop new criteria analysis. 5 babies between 9 and 16 weeks old were filmed in free movements for 15 minutes in car seat, cocoon and on the floor. Videos have been analyzed independantly by two examiners and once more analyzed later by each of them. The reliability of the results obtained with this software has been checked thanks to inter and intra-examiner analysis. ELAN is a simple and reliable tool to analyse the movements of the child. It will be soon used to compare first representations of orality in different devices.
