Analyse qualitative d'un inventaire guidant le choix et l'adaptation d'un dispositif de communication alternative et améliorée (CAA), convenant le mieux à l'enfant ou à l'adolescent non verbal avec un trouble du spectre de l'autisme ainsi qu'à son environnement ENVIRONNEMENT

Les Troubles du Spectre Autistique englobent un large éventail de troubles de la communication sociale et des comportements, restreints et répétitifs. Des difficultés langagières parfois importantes, requièrent une intervention précoce dans le domaine de la Communication Alternative et Améliorée. Celle-ci nécessite une évaluation préalable, tenant compte des capacités, des spécificités de la personne et des caractéristiques de son environnement. Un inventaire, le Y’a CAA, a été conçu pour synthétiser ces éléments, afin de choisir, d’adapter et d’installer un dispositif de CAA. Cet inventaire a été complété pour huit enfants et adolescents non verbaux, avec autisme. Ils étaient suivis par des professionnels de formations diverses et avec plus ou moins une utilisation préalable de la CAA. Quatre types d’établissements de soins étaient représentés. Nous supposions que le Y’a CAA permettrait le recensement des informations indispensables au choix et à l’installation d’une CAA. Aussi, nous présagions qu’il serait pratique et facile d’utilisation pour les orthophonistes, quel que soit leur mode d’exercice. L’analyse des données a montré la pertinence des items du Y’a CAA, permettant de recenser des critères inhérents à l’enfant et à son environnement. Toutefois, certains items étaient plus ou moins accessibles et faciles à renseigner selon la situation institutionnelle et familiale de l’enfant. Aussi, certains critères spécifiques, rapportés dans la littérature, se sont révélés absents de l’inventaire. L’analyse de la concordance des réponses des partenaires sociaux, a prouvé l’intérêt d’une co-élaboration de l’inventaire, pour obtenir une vision globale de l’enfant. Par ailleurs, nous avons soulevé l’importance d’informer le grand public sur la CAA et de réfléchir sur la formation des professionnels à la modélisation du dispositif. L’intérêt d’intégrer des essais techniques au Y’a CAA, a été souligné. Cet inventaire pourrait faire l’objet d’une autre étude, permettant de le parfaire puis de le valider sur le plan statistique.

The autism spectrum disorder encompasses a wide range of social-communication impairments and restricted and repetitive behaviors. Sometimes, important language difficulties require for early Alternative and Augmentative Communication intervention. This one demands a prior assessment, taking into account the person’s capabilities, specificities and the characteristics of her environment. An inventory, the Y'a CAA, was designed to synthesize these elements, in order to choose, adapt and install an AAC device. This inventory was completed for eight nonverbal children and teenagers, with autism. They were followed by professionals of various vocational training and with more or less a prior use of the CAA. Four care facilities types were represented. We assume that the Y’a CAA would allow the census of all essential information to the selection and installation of an AAC. Also, we presume that this tool would be practical and easy to use for speech and language therapists, whatever their mode of exercise. The data analysis showed the relevance of Y'a CAA items, making it possible to identify child and his environment inherent criteria. However, some items were more or less accessible and easy to inform depending on the child's institutional and family situation. Moreover, some specific criteria reported in the literature, were found missing from the inventory. The concordance analysis of the social partners answers, proved the interest of a co-elaboration of the inventory, to obtain a global vision of the child. Otherwise, we have raised the importance of informing the general public about AAC and to think about the training of professionals in modeling AAC device use. The interest of integrating technical tests in the Y'a CAA, was emphasized. This inventory could be the subject of another study, allowing to perfect it and then to validate it statistically.
